This form needs to be filled per group / per class by respective Choreographers / Class teachers. Parents do not need to fill this form for their students participation in pathshala class performance.

PROGRAM GUIDELINES
1. Performance Item should be no more than 5 minutes (except for a drama-special permission is required for any item longer than 5 minutes in duration).
2. No event shall have less than five (5) individuals.
3. No duplication of events (no Stavan, Stuti, Bhajan, or Dance should be repeated). For duplicate audios, selection will be based on first come first serve.
4. The sequence of items will be declared on the day of the event.
5. No glass bangles will be allowed on stage or dressing room for safety.  No Colors, Objects, paper, confetti, grains, particles, or matter of any nature whatsoever will be sprinkled on or thrown on stage or dressing room or in any other place.
6. No Community hours will be awarded for practice/performance in the MAHAVIR JANMA KALYANAK CULTURAL PROGRAM.
7. Performance Item should be fully rehearsed and be ready with the complete program.
8. A participant cannot participate in more than two (2) events, as we want to give other individuals a chance to participate.
9. The event director (teacher/parent/choreographer) should be informed that the stage set up is their responsibility.
10.   If a student misses two or more pathshala class practice sessions, their participation in the class performance will be at the discretion of the respective class teacher.


Our group participants and I have read the guidelines.  We understand and agree to abide by them and help JAIN SOCIETY OF METROPOLITAN WASHINGTON for a smoother program.

MJK committee reserves the final decision on approving the application for year 2019 Mahavir Janma Kalyanak program.
